The exam preparation problem nobody talks about
Medical examinations are unlike any other professional assessment. They do not test whether you can memorise a fact — they test whether you can retrieve the right fact, from the right source, in the right clinical context, under time pressure. The volume of material is staggering. A urology trainee preparing for the European Board exam faces the entire EAU Guidelines (over 1,500 pages across multiple chapters), Campbell-Walsh-Wein (four volumes), plus supplementary journal articles and institutional protocols. A general surgery trainee preparing for MRCS faces a similar mountain across anatomy, physiology, pathology, and surgical sciences.
The traditional approach to managing this volume has not changed in decades. You read the material once, make notes, create flashcards, and then cycle through question banks until the patterns stick. Anki decks and spaced repetition have become the gold standard for memorisation. Question banks like Pastest and UKMLA practice papers test recall under exam conditions. These tools work — for a specific type of learning.
But here is the gap that nobody addresses. When you get a question wrong, or when you encounter a clinical scenario that does not match the patterns you have memorised, you need to go back to the source material and understand why. Why is radical cystectomy the standard of care for muscle-invasive bladder cancer? What does the EAU guideline actually say about neoadjuvant chemotherapy? What is the level of evidence? And critically — where exactly in the 200-page chapter does it say that?
This is where the traditional toolkit fails. Anki tells you the answer, not the reasoning. Question banks test recall, not comprehension. And when you need to trace an answer back to its source, you are back to flipping through PDFs, searching for keywords that may or may not match the phrasing the authors used, and losing twenty minutes to find a single paragraph.
Why general AI tools make this worse
The instinct, when faced with a knowledge gap during exam preparation, is increasingly to ask ChatGPT or Gemini. It feels efficient. You type a question, you get an answer in seconds. But for exam preparation specifically, this approach has a fundamental problem: the answer is not traceable to your study materials.
When a general AI tool tells you that neoadjuvant cisplatin-based chemotherapy is recommended for T2-T4a bladder cancer, that may be correct. But which guideline is it citing? The EAU guidelines? The AUA/ASCO/SUO joint guideline? The NCCN? These guidelines occasionally disagree on specifics — on the strength of recommendation, on the eligible population, on the preferred regimen. If you are preparing for a European exam, you need the EAU recommendation specifically. If your institution follows NCCN, you need that one.
General AI tools cannot make this distinction because they do not have your study materials. They answer from a blend of training data that includes medical websites, abstracts, patient-facing summaries, and fragments of guidelines. The answer may be directionally correct but lacks the specificity that examiners expect. In a viva, "the guidelines recommend it" is not the same as "the EAU 2025 Guidelines recommend cisplatin-based neoadjuvant chemotherapy with a strong recommendation, Level 1a evidence, as stated in Chapter 7.5.2."
Examiners do not test whether you know the general answer. They test whether you know which source says what, and whether you can defend your answer with specific evidence. General AI tools cannot give you that.
A different approach: your documents, your queries
The approach I want to describe is conceptually simple but practically powerful. Instead of asking an AI tool that draws from the entire internet, you upload your actual study materials — the specific guideline editions, textbook chapters, and journal articles that your exam syllabus requires — and query them directly.

Start with a question bank. Work through a set of practice questions in your target exam format. When you get a question wrong — or when you get it right but are not confident in the reasoning — note the topic and the specific knowledge gap.

Medical exams, particularly clinical vivas and OSCEs, frequently test knowledge that lives in figures and tables rather than in running text. Treatment algorithms, staging systems, diagnostic flowcharts, and management pathways are all presented visually in guidelines and textbooks. If your AI tool only indexes text, it misses half the exam-relevant content.

AI helps with the former, not the latter. Ward rounds, operating lists, and clinic exposure remain non-negotiable.
Use AI to build and verify your knowledge base efficiently. Use practice questions to test retrieval under exam conditions. Use clinical experience to develop the judgement that examiners are actually assessing. All three are necessary — none is sufficient alone.
A note for supervisors and medical educators
If you are a consultant supervising trainees, this approach has an additional benefit. You can create a collection of the materials you consider essential for a rotation or exam, share the collection name with your trainees, and have them upload the same materials. This creates a shared evidence base — everyone is studying from the same sources, and when a trainee asks a question, you can direct them to query their collection rather than sending them to Google.

Too often, medical education relies on distilled summaries — lecture slides, review articles, question bank explanations — that are two or three steps removed from the original guideline or study.
